Complete a brief medical history and review our patient agreement. This will take about 15 minutes and can be most easily completed on a laptop or desktop computer.
2. Share Your Medical Records
Help us prepare for your first visit by requesting records from your current providers.
3. Meet Your Physician
Once you’ve completed enrollment, we’ll invite you to schedule your first appointment—an unhurried (45-60 minutes), in-depth visit where we get to know you, your history, and your goals. This is the foundation of your personalized care plan.
4. Enjoy Direct Access & Seamless Care
Text, call, or video chat with your physician whenever you have questions. You will have ongoing support throughout your health journey.
